Tok on the Limits of Knowledge Essay â€Å"the vocabulary we have does more than communicate our knowledge; it shapes what we can know’. Evaluate this claim with reference to different areas of knowledge. † At first glance this title seems to be ridiculous. Our vocabulary can have complete control over what we can know’. It seems unlikely that there is knowledge that cannot be obtained due to the limits of our vocabulary. However when you look at this in depth it seems to be true. What is vocabulary? Vocabulary are the words we use. Vocabulary has a connection to the word itself and what’s behind it. Say the word cow. The word â€Å"cow† is merely just a noise. But we automatically connect this word to the animal. Every word has this â€Å"magical touch† to it. Thinking about vocabulary this way we can begin to see that knowledge is shaped by vocabulary because the words we have have individual meanings behind them. Without any vocabulary to represent unfound knowledge is there no way for it to be discovered? Different areas of knowledge argue against this idea. Human sciences allow us to perceive and have different observations of vocabulary. If vocabulary can hold different meanings and perceptions to different people and cultures this could potentially allow knowledge to break past the barrier of language. Language also holds its own set of rules. These rules (Context, comprehension, grammar ) can alter the meanings of words to once again allow for different meanings. Language seems to be solid and rule bound. But this is not the case, Language is creative and open-ended. One problem it is faced with is that what one person means when they say something may not be what another person understands when they hear it. This allows for millions of different interpretations. interpretation is very important to the understanding of knowledge. Knowledge runs hand in hand with language, If we fail to interpret On the basis, a general understood meaning allows us to maintain knowledge whereas these new meaning can help us discover knowledge. Language surrounds us wherever we go yet we fail to realize it, yet again it plays one of the biggest roles in human life. We use language in almost everything we do. Language is so important because it allows us to break out of our own self knowledge and enter into the minds and understandings of thers through communication. Communication is the foundation of knowledge. But communication has its flaws that allow knowledge to become hazy and hard to acquire. One example is what one person says might not be what another person understands. This is a huge barrier in communication, which correlates to a huge barrier in knowledge. Think of trying to learn something and h aving no background or understanding of the vocabulary used. You are not able to understand this knowledge because you don’t understand meaning. Meaning is the basis to understanding vocabulary. So to not understand meaning of vocabulary, we cannot learn knowledge. This is one way vocabulary begins to limit what we can know. You might think an easy solution to this problem would be to just consult a dictionary or ask for a further explanation of the unfamiliar vocabulary. Although this seems promising and easy, we do not always get the result we look for. Communication has its way of being interpreted. Different people and different cultures have their own understandings of words. Words have fuzzy undefined edges to them. Try explaining the word love. Without much thought you can understand that what it means to you could be completely different to someone else. Looking at vocabulary this way you can see that it creates a viscous cycle that cannot be conquered. Another problem similar to this one is that vocabulary has words that have something to show for them and some that don’t (denotation theory). An example I used earlier is cow, You can look at a cow. Words like these help communication which makes knowledge seem to have fewer limits, But its words like wisdom, pride, or freedom that bring up issues in understanding knowledge. Look at the word pride. You can look at examples of pride, but it does not represent one singular thing that is understood by everyone everywhere. Once again this creates the viscous cycle I mentioned earlier, Until we learn to have a fundamentally sound definition for these words like these will vocabulary no longer limit our knowledge. Now that we have looked at some problems of vocabulary itself we can begin to apply the problems found in communication that limit what we can know. The poet Robert frost observed that rarely do humans ever say exactly what they mean. What he means is that humans during communication will include five problematic meanings. These five are vagueness, Ambiguity, secondary meanings, metaphor, and irony. These five problematic are a huge limitation on communication, if they are not immediately picked up on during communication our knowledge is greatly impacted. First is vagueness. Consider the word tall. While taken out of context this word has infinite possibilities. Tall means completely different things while talking about how tall you are or talking about how big buildings are in major cities.

Diagnosis and Management of Bronchopleural Fistula

Diagnosis and Management of Bronchopleural Fistula Introduction A bronchopleural fistula (BPF) is a communication between the bronchial tree and the pleural space. More than two thirds of BPFs are postresectional, with an incidence ranging from 1.5-28 % after pneumonectomy, and 0.5% after lobectomy. Postresectional BPF is associated with mortality of 20-50 %.[1] BPFs can present early (1-7 days), intermediate (8-30 days), or late (more than 30 days) after pulmonary resection. Early fistulas are usually associated with dehiscence of the bronchial stump, while late fistulas are usually associated with failed healing or infection.[2] Besides postresectional etiology, BPFs may be idiopathic, or secondary to necrotizing lung infections, persistent spontaneous pneumothorax, thoracic trauma, Adult respiratory distress syndrome, Tuberculosis, or postprocedural e.g. lung biopsy or bronchoscopy.[3] Diagnosis and management of BPF is a major challenge for clinicians. Diagnosis and localization of BPF may require multiple chest computed tomography (CT) imaging and bronchoscopies.[4] A BPF is usually a manifestation of the underlying lung disease, therefore, measures to reduce the air leak are generally unsuccessful until the patients underlying lung disease has improved. Once there is diminished need for mechanical ventilation, the BPF almost always resolves without specific therapy, apart from controlling active infection and draining the pleural space adequately. Definitive repair using surgery or various bronchoscopic methods has been used to close BPF if either conservative management fails, or is not expected to succeed.[5] Case Description A 69 year old male with squamous cell carcinoma of the right lower lobe (RLL) of the lung underwent thoracotomy and RLL resection. The patient was discharged home on the 6th postoperative day after an uneventful hospital course. The patient presented to the emergency department of an outside hospital ten days later with fever and respiratory distress. Chest X-ray showed a large pneumothorax, and a right sided chest tube was placed. Pleural fluid was sent for culture and sensitivity, and later grew staphylococcus aureus. The patient’s breathing continued to be labored, and was intubated urgently after failing a short trial of non-invasive positive pressure ventilation (PPV). A large air leak was observed through the chest tube with each positive pressure breath, even after placing the chest tube on continuous suction. Placement of an endobronchial valve to minimize air leak was attempted in the interventional bronchoscopy suite, but failed and was reported to be technically dif ficult. The patient was subsequently transferred to our hospital for further management. On admission, ventilatory management in the intensive care unit (ICU) was geared towards lowering the tidal volume and plateau airway pressure to minimize the air leak. This was accomplished by providing partial ventilatory support using low rate synchronized intermittent mandatory ventilation (SIMV) and a high inspiratory flow rate, as well as decreasing PEEP, inspiratory to expiratory (I/E) ratio, and the tidal volume of each ventilator delivered breath. We used the least amount of chest tube suction that maintains lung inflation, treated bronchospasm aggressively, and continued appropriate antibiotics. We suspected that the patient had a RLL airway stump dehiscence resulting in a BPF, and complicated with staphylococcus aureus empyema. Chest CT (Figure 1) showed a properly placed right sided chest tube surrounded by residual hydropneumothorax. Bronchoscopy showed a defect in the suture line of the RLL airway stump. The patient was taken to the operating room (OR) to close the BPF through a right thoracotomy after switching the single lumen endotracheal tube to a left sided double lumen tube. The abscess cavity was cleaned revealing a 0.6 cm airway stump opening at its base. The stump opening was sutured and mattressed with intercostal muscle pedicle. A Clagett window was created, the cavity was packed tightly, and a vacuum assisted closure dressing was applied. The patient was extubated in the OR to minimize PPV. Antibiotics and dressing changes were done until the patient was discharged from the hospital on the 6th postoperative day. The wound was ultimately closed five months later when the Clagett window decreased in size and healed appropriately. Diagnosis Signs and symptoms of BPF include fever, productive cough, hemoptysis, subcutaneous emphysema, and persistent air leak or purulent drainage from a chest tube. Late fistulas with empyema may be associated with nonspecific symptoms e.g. fever, cough, and shortness of breath. Chest radiographs usually demonstrate a new air-fluid level (hydropneumothorax) in the pleural space.[6] Both chest CT and bronchoscopy are useful in establishing diagnosis, identifying the cause and localizing the bronchopleural fistulous track.[4] Chest CT can also define the empyema cavity and assess the severity of coexisting lung disease. Bronchoscopy can also be used to inspect the bronchial stump, obtain cultures, and assess the contralateral airway. In addition to direct visualization of central BPFs, bronchoscopy can localize peripheral ones by passing a balloon into suspected bronchial segments separately and then inflating for 30-120 seconds. A reduction in air leak through chest tubes would identify the bronchial segment communicating with the BPF.[7] Management Management of BPF is difficult and depends on the underlying cardiopulmonary reserve, nutritional status, fitness to undergo a major surgical procedure, surgical expertise available, and the size and location of the fistula. Conservative management: The primary goals of BPF management include treating the underlying lung disease, reducing the air leak through ventilator adjustments to minimize the tidal volume and plateau airway pressure, and avoiding contamination of the contralateral lung by either positioning the patient with the affected side down or controlling the airway with a double lumen endotracheal tube if needed. Small fistulas can be managed conservatively if the patients clinical condition is stable and the pleural space is well drained. This is achieved by insertion of a chest tube in the pleural cavity, with either minimal or no suction to allow the lung to expand without encouraging air leak through the fistula. Conservative management is safe as long as empyema is drained promptly, but might not be suitable for patients requiring prolonged mechanical ventilation.[8] The chest tube can be connected to a one-way valve and the patient can be treated as an outpatient.[9] Ventilatory management: [9,10] Ventilatory management of BPF is geared towards keeping airway pressures below the critical opening pressure of the fistula by reducing the respiratory rate, tidal volume, inspiratory time, PEEP, plateau airway pressure, and transpulmonary pressure gradient. This can be achieved by implementing the following measures: Provide partial ventilatory support e.g. low rate SIMV or Pressure Support. Decrease the tidal volume of each ventilator-delivered breath (5 to 8 mL/kg) Use a high inspiratory flow rate e.g. 70 to 100 L/min. Decrease I/E ratio. Minimize inspiratory time to decrease mean airway pressure, avoid end-inspiratory pause and inverse-ratio ventilation. Minimize PEEP (both dialed-in and auto-PEEP). If previous adjustments are not possible or insufficient, a strategy of permissive hypercapnia is appropriate (decrease minute ventilation and allow arterial PCO2 to rise). Use the least amount of chest tube suction that maintains lung inflation. Avoid patient positions that increase the leak, and keep patient heavily sedated (and paralyzed if needed) if spontaneous movement exacerbates air leak. Treat bronchospasm and other causes of expiratory airflow obstruction. Wean and extubate patients as soon as possible. If the measures mentioned above failed, consider unconventional ventilatory measures e.g. independent lung ventilation,[11] high-frequency jet ventilation,[12] or airway pressure release ventilation.[13] If ventilation continues to be problematic, veno-venous extracorporeal membrane oxygenation (ECMO) can be used temporarily until further bronchoscopic or surgical control of BPF is done.[14] Bronchoscopic Management: Interventional bronchoscopy is usually reserved for BPF in patients with high surgical risk and poor general condition, or small fistulas in stable patients after drainage of the pleural space has been achieved and infection is controlled.[15] These procedures are limited to isolated case reports or short case series; the current literature does not allow adequate comparison of different bronchoscopic techniques. Peripheral fistulas (post-lobectomy) tend to be more amenable to this family of techniques compared with more centrally located fistulas (post-pneumonectomy). Bronchoscopic management of BPF is based on the delivery of different materials and small devices into the BPF sites.[16] Examples include: A plug composed of Gel Foam, fibrin, autologous clot, or tissue glue. An endobronchial one-way valve. (Figure 2 3) An expandable endobronchial stent. Local application of silver nitrate. Coagulation of the injured site with the Nd:YAG laser. Antibiotics e.g. Doxycycline, or tetracycline injections. Silicone fillers, coils, and amplatzer devices. Surgical Management: Surgical management has a success rate of 80-95 %, but it is associated with high morbidity and mortality. Surgery can be performed as a one or two stage procedure. A one stage procedure is indicated when the BPF occurs within the first few days after surgery. Management usually includes closure of the BPF by reclosure of the bronchial stump reinforced with omental or pedicled flaps, and obliteration of the residual pleural space.[2,17] Management of late occurring fistulas or if an empyema is present involves a two-stage approach. Drainage of the pleural space and debridement of empyema cavity is followed by repair of the air leak once the pleural space is sterile (e.g. Modified Clagett Technique).[18,19] Chronic empyema may need thoracoplasty with removal of a part of the chest wall.[5] If the BPF cannot be identified, open window thoracotomy and daily dressing may be helpful. Success of surgical management is improved by ensuring appropriate antibiotic therapy, adequate nutritional support, and proper care of chest tubes, as inadvertent occlusion can be complicated by tension pneumothorax.[8] Conclusion BPFs offer many management challenges in a complex patient population. The aforementioned case report demonstrates the spectrum of morbidity associated with this serious problem. A post-resection occurrence is the most common etiology, and the diagnosis is usually confirmed with chest CT and bronchoscopy. BPFs require a multimodal management strategy and should be handled in centers with experience with this complex problem. Management includes control of infection, pleural drainage, proper pain control, nutritional support, and ventilatory management tailored to minimize air leak. Both surgical and non-surgical options can be utilized for management of BPFs. Bronchoscopic procedures such as endobronchial valves are emerging as viable treatment options but should be used on a case-by-case basis. Mindful Destruction of Order in Shakespeares Othello Essay -- Shakespe

A delicate balance of chaos and order exists in our lives; the balance maintains itself by the very acts of human nature. In the drama Othello, the battle between good and evil creates the basic root of human nature as a whole. While at first, order exists in the lives of the Othello and Iago, through dramatic events and manipulation, the balance becomes unstable and starts to shift into chaos. Once the chaos has started, it continues a chain that continues along until the very end where the balance finally restores itself as it would in human nature. Through the inter and intra personal dialogue between Othello and Iago, a certain image of the character becomes developed because of the continued additions and changes to the character’s situation. Seen as the stronger of the two, Othello holds less intelligence in common matters. Iago’s character shapes out to as more of cunning and strong-headed. Shakespeare uses the characters to effectively reaffirm the basic traits of human nature and show how the traits coincide with the theory of order and chaos. Thorough the use of extensive characterization in this drama, along with artful diction, the development of Othello and Iago, represents a view of humans and their lives along with the all too classic good and evil. Iago represents evil at the most simplistic level. Of the many traits Iago has ascertained many represent the hatred and jealousy that he has for the people he manipulates, â€Å"But for my sport and profit. I hate the Moor...[he] is of a free and open nature. That thinks men honest that but seem to be so. And will as tenderly be led by th’ nose. As asses are,† (1.3. 377-393). Iago knows that Othello has a trusting nature with most men who appear honest, he knows he has th... ...hat exactly happens between the point of believing and realizing the truth. From the play, one grasps an understanding of the crucial need to think and look at the facts in life before acting on an impulse. It also shows that chaos will always exist no matter what, but if we do not understand it, we cannot make embrace it. Through Othello’s lesson, we learn that the truth can always remained covered up and not represented entirely, and therefore it becomes our own responsibility to take the lead and grasp the understanding of things. The Importance of Foreign Language Education The main goal of learning a new language is to be able to communicate in that language. The ERIC database’s thesaurus defines language proficiency as the capacity of a person to accurately and fluently communicate using language (Language Proficiency, 2004). While gaining this ability is a main reason for studying a foreign language, there are many other reasons why everyone should take the time to do so. Occupational, cultural and developmental benefits are some of the most prominent ones to be had. Occupational benefits are perhaps the most important and widely known ones associated with language learning. When a person speaks more than one language, they are usually highly valued by employers. Favor in an employer’s eyes can obviously lead to desirable promotions and salary increases for multilingual workers. A CNN writer reported, â€Å"When a manager decides whether to promote on of two people with identical technical skills, the better communicator is more likely to get the nod† (Hall, 2001). Companies, especially ones with international operations, like to have a staff that is able to interact with partners and associates in overseas markets, as well as multinational clients here at home in the United States. This allows them to create and maintain positive relationships and increase revenue. One authority affirms the nature of such companies by saying that many of the operations of international corporations occur on a on a truly global scale (Gutek, 1993, p. 233). For companies that do not have global operations, hiring a staff that is multilingual is sure to facilitate the creation of a profitable international department. Wedding Speech Delivered by the Groom I would like to start my speech today by thanking Richard for his kind words. I am really proud to have become Isia’s husband; ever since the day we met I have been treated as one of the family and this has made our Journey to this day that much smoother! Thank you both for your blessing I promise that I will make you proud and live up to the high expectations I am sure you have! Thank you for everything you have both done in making this day so special if it wasn’t for you I am sure I would still be saving. So far the day has been an amazing experience and I am sure it can only get better. I would also like to thank you on behalf of Isia for all the love and help you have given her over the years. I thank you for making Isia the most loving, caring, sweet and kind person I have ever met. I really am the luckiest man alive. Thanks to Mom and Dad: I would now like to thank my own mom and dad for all the love and support they have given me over the years. I am sure you are looking at me now and thinking what a brave boy? I am not really one for speeches especially in front of 80+ people. You have always been there for me and helped me though good times and bad especially in my younger days. If I have ever had a problem where did I go mom and dad of course. I couldn’t ask for two more loving people. I am very proud to have you here with Isia and I on our special day. I know that your love and support will continue for many years to come. Thanks to Isia: I now come to the most important person in my life, and that’s Isia my beautiful bride. I am so proud to say that you are my wife you really do look amazing. I am sure that you will all agree that she has done me proud. I wrote this before today so I never knew until now just how amazing you would look. Knowing you like I do I am sure you will now be getting rather embarrassed and possibly a little pink. But I am sorry I must go on. We first met at a new years eve party in 1999 we said hello and that was it.

Time Management Research Strategy Paper At first, I thought the example given; â€Å"Time Management† was not a good example to use for my paper. I didn’t really [Clearer writing suggestion–â€Å"real† or â€Å"really† means â€Å"existing in actuality†Ã¢â‚¬â€œit adds little to the meaning (and using it to mean â€Å"big,† â€Å"very,† or â€Å"genuine† is slang); replace it with a more expressive word] feel like I have an issue managing time, after all I complete my task about ninety [Express numbers higher than nine in digits (when not the first word in the sentence)] -nine percent of the time and normally finish on or ahead of schedule. However, when I stopped to look at what I complete I was surprised [The passive voice is a form of â€Å"be† (was) and a participle (surprised). Over-use of the passive voice can make paragraphs tedious to read and officious. Try to use the active voice most often, e. g. , the student completed the paper on time. The passive voice version–The paper was completed on time by the student–See eCampus>CWE>Tutorials & Guides>Grammar & Writing Guides>Active & passive voice] at my findings. I found that I have to some degree, mentally categorized my task. What I mean by this is that I have, without thinking about it, categorized my task into two separate groups: Work and personal. The first, being my primary group is the tasks related to my job. This group boasts has a very high completion rate and quality of work on these tasks is checked [Passive voice] by my employer as well and then communicated to me. Based on task tracking I have about a ninety [Express numbers higher than nine in digits (when not the first word in the sentence)] -eight percent completion rate and based on feedback from my employer I know the quality of my work is very high as well. Now, the second category is my personal life category and my after examining it, I found that I don’t have results anywhere near my what I have in my job related category. My personal category breaks down into two groups as well: The have to be done now [Clearer writing suggestion–â€Å"Now† is a tricky concept. If the sentence is in the past tense, it probably should be rendered as â€Å"then. † If not, and unless you are making a philosophical statement (Now is the time to improve myself) or mean â€Å"as of the present time† (the business is now known as Ajax Corporation), onsider removing â€Å"now†Ã¢â‚¬â€œbecause this is being read after the time you wrote it, your â€Å"now† is in the past] and the can wait until later groups. I find that when I leave work, I leave a task-oriented environment and enter an environment of relaxation and enjoyment. This environment of less critical tasks allows the task to be ignored [Passive voice] until they can’t be ignored [Passive voice] any longer, forcing some sort of action. Now in review of this method, the completion rate is not the only item to score poorly, the quality of the work suffers tremendously as well. Project oriented tasks that have some sort of visible outcome; such as [Check word choice: â€Å"such as† refers to things that are exactly what you are discussing; â€Å"like† means something similar to what you are discussing] laying tile in a walkway get [Doctoral rule (although good advice for any academic writer)–â€Å"get† is informal English and can mean many things; in academic writing, use forms of â€Å"arrive at,† â€Å"can,† â€Å"could†, â€Å"grows,† â€Å"is able to,† etc. ] completed in a timely manner and with a much higher level of quality. My goal for this research paper is to develop a strategy for improving my time management skills for my personal category. With the proper time management skills I believe the quality of work on these tasks will also rise exponentially. My goal requires information on time management tools, prioritization and [Check punctuation–insert a comma before this word if this is the last in a list of more than two–or if it begins a new clause] organization. I want to gather information from credible sources, so I will be looking for information from reputable time management coaches. Additionally I need to answer the following questions: â€Å"How can I improve my prioritization skills? What organization tools are there for time management? and â€Å"What are the keys to managing your time? † I will use the Internet for my research because it can be accessed 24/7 and reference any books that are available through â€Å"University of Phoenix† online library. I will place the information retrieved into two categories, relevant and non-relevant. Discarding the irrelevant data and sorting the relevant into categories that fit the questions that I have asked and a category for the questions that may be applicable which [Use â€Å"that† for a restrictive phrase (or place a comma before â€Å"which†)] I did not ask. I will then analyze the data, re-categorizing if needed, examine the evidence of recommendations, determining whether the information is fact or opinion and if it is opinion is it supported [Passive voice] by any facts. I will make comparisons to similar information identify any themes or ideas and align the information with other information retrieved. Then I will examine perspectives and assumptions. By determining what perspectives the author [If this means yourself, avoid referring to yourself in the third person; if this is a personal account, use the first person (I, me, my)] may have I will derive a better idea of what direction his information is coming from and better determine how applicable it is to my situation. Likewise, if the author has made or not made certain assumptions the information collected from this person may be applicable to my situation. There may also be a need to prove or disprove an assumption to determine just how relevant a certain piece of information is or for that matter how relevant a certain author is. I think it is important to examine my perspective as well as the author’s so that I can be more specific in my search for answers. As research progresses and more is learned [Passive voice] about time management, I will ask additional questions and look for examples of these ideas and tools played out in everyday life to ensure its suitable to me. The information must make sense and be adaptable to anyone looking to improve his or her time management skills, but I will also be looking for ideas and tools that fit my character and lifestyle. Chemistry

Mixture separation Lab I am one of the chemists assigned to design a cost effective procedure for handling the separation and recovery and hazardous waste disposal. The difficulties were trying to figure out Is how we can effectively separate Carbons, sand, and naphthalene? The hypothesis we chose to solve this problem was If we put the mixture into water, alcohol, and mineral spirit, then in each solution one or more substance will not dissolve, will be visible, and also able to separate.According to our data our hypothesis supported the experiment because and sand are lobule in water while naphthalene is not. The CARBONS, and sand will dissolve in the liquid solvent and the naphthalene will remain in the solid phase. In the other substances methanol and mineral spirit were not soluble in both Carbons and sand. Also in naphthalene mineral spirit was not soluble. To accomplish this experiment we did the following as to have a successful hypothesis.First use three separate ml test tub e, and added ml of each substance Including water, methanol, and mineral split. If the Carbons Is something that Is water soluble, then water can be added to the mixture and then It can be altered as the sand is insoluble and will be left behind. Also add 0. 20 grams of each mixture including Carbons, naphthalene, and sand to each test tube. Try to stir or shake the substance to dissolve.If that didn't work to dissolve the mixture we made a filter paper and placed it in a funnel then try to filter it by pouring the sample in. After that this leads us on to the third option of separation which is evaporation. At last we placed our sample test in an Erlenmeyer flask and then placed it on a hot surface with melting point 8. 2 0 c, waited to evaporate then recorded our data. After each test, try to observe If the sample was soluble In any of the substance.My data definitely supported my hypothesis because In that experience we found out In the lab that H2O was not soluble In naphthalene , and sand. Methanol was not soluble in Carbons, and sand. Also mineral spirit was not soluble in all the substances. Some errors that might have affected my experiment was the evaporation state I think we left it a little bit too long or took it our earlier for the solution. In the filtering stage we might have spilled some of the sample result so hat's why we maybe got different result.
